Print an Entry Form (entry form must be printed on one page, portrait mode), complete, sign, and mail (with check or money order to GNA Futbol Club for the appropriate amount to:

• To be considered on-time, Entry Forms must be received by April 9, 2010.
  First Class Postage is now 44 cents.

• Notification of acceptance or non-acceptance will be sent no later than April 16, 2010.

We appreciate your interest in the GNA Junior Cup. However, inclement weather is always a possibility and such weather may result in abbreviated or cancelled games. In the event of tournament cancellation due to inclement weather, there is no guarantee of a refund.
